By The Health Stacks Research TeamUpdated September 4, 2026

Exendin-4 is a small protein-like substance called a peptide. It was first found in the spit of a Gila monster lizard. It works much like a natural body hormone called GLP-1, which helps control blood sugar after meals. Scientists study it a lot, and a lab-made copy of it is used as a diabetes medicine in some places. Exendin-4 is good at turning on a certain switch on cells called the GLP-1 receptor.

When this switch is turned on, the body releases more insulin only when blood sugar is high, and the stomach empties more slowly. This can lower blood sugar. In labs, it also seems to help certain healing cells, like ones that build tendons, grow the right way. It lasts much longer in the body than the natural hormone, which is why researchers like it. Scientists are still learning about this peptide.

Studies suggest it may help with blood sugar control, weight, wound healing, and even tendon problems, but much of this work is early or done in animals. Like similar medicines, it can cause stomach upset such as nausea. More research in people is needed before we know all of its good and bad effects. Anyone thinking about using it should talk to a doctor first.

Research-stageEarly-stage — evidence so far is mostly cell, animal, or preliminary human studies. Not an approved medicine. High confidence60–79 — backed by solid sources with good corroboration across multiple citations.

mg vs mcg: one milligram (mg) equals 1,000 micrograms (mcg) — multiply by 1,000 going from mg to mcg, divide by 1,000 going from mcg to mg. Mixing the two up is a thousand-fold dosing error: 1 mg taken instead of 1 mcg is a 1,000× overdose; 1 mcg instead of 1 mg is a 1,000× underdose. Always double-check the unit label and the decimal place.
