The BELIEVE randomized phase 2 trial tested the ActRII-targeting muscle-preserving antibody bimagrumab alone and combined with semaglutide in adults with obesity over 72 weeks. At week 48, lean mass rose +1.0% to +1.1% with bimagrumab versus -4.7% to -6.9% with semaglutide. At week 72, the high-dose combination cut total body fat mass by -45.7% while largely preserving lean mass.
